A multi-facility healthcare provider network struggled to gain a complete view of patient populations because critical information was spread across electronic health records, claims systems, laboratory platforms, and remote monitoring devices. Clinical and operational teams lacked a single trusted source of information, making it difficult to monitor chronic diseases and coordinate preventive care.
Reporting relied heavily on manual spreadsheet consolidation, consuming valuable analyst time while delaying access to actionable insights. Healthcare leaders could not identify regional disease patterns or at-risk patient groups quickly enough to support proactive interventions.
The organization needed a modern analytics platform capable of integrating diverse healthcare data sources, improving reporting speed, and establishing a governed foundation for future analytics and AI initiatives.

Our Approach
Softree designed and implemented a scalable population health analytics platform using Microsoft Fabric as the central analytics environment, OneLake as the unified data repository, and Power BI for interactive reporting. Data from EHR systems, claims platforms, laboratory systems, and wearable devices was securely ingested into a governed lakehouse architecture following Bronze, Silver, and Gold data layers.
The implementation followed a phased delivery approach. Secure data connections were established first, followed by automated ingestion pipelines, data cleansing and standardization, semantic data modeling, and finally the deployment of role-based dashboards for clinicians, executives, analysts, and care managers.
The platform delivered dedicated dashboards for diabetes trends, hypertension monitoring, heart disease risk assessment, preventive care, and vaccination coverage, enabling healthcare teams to monitor population health through a single source of truth.

The Outcome
Within two quarters of deployment, the healthcare provider significantly improved how clinical and executive teams monitored patient populations. Manual reporting effort was reduced by 78%, while clinical teams gained insights four times faster than previous reporting processes. The platform provided visibility into vaccination coverage across the population and supported analytics for more than 428,000 patients.
Near real-time reporting enabled earlier identification of at-risk patient groups, improved preventive care planning, and eliminated time-consuming spreadsheet-based reporting.
